NSAA Pre-Participation Physical Examinations
Physical Examinations for the 2020-2021 school year will be required for students entering 7th and 9th grade this year. Returning students participating in an activity or physical education class for the 2020-2021 school year will be able to use their physical examination form from the 2019-2020 school year. The Student-Parent Consent form will still need to be completed and returned to the office or the coach/sponsor prior to participating in practice. Physical examinations completed after May 1st will be accepted. Students will need to schedule an appointment with a clinic of their choice and return the proper paperwork to OHS in the fall. There will be a cash option and an insurance option at most facilities, more information will be available when you schedule your appointment.
